TradeWinds Wavier Case Management
- Develops goals that are consumer-based while promoting personal growth
- Insures that all Individualized Support Plan (ISP) goals are being implemented and measured for progress by providers
- Insures that each consumer is free from abuse, neglect and exploitation
- Insures that each consumer is ALWAYS treated with respect and dignity
- Face to face visits with consumers at least every 45 days
- Strong moral and ethical values
All Waiver Case Managers are state approved Person-Centered
Planning/Individual Support Plan facilitators. Thorough
understanding of the waiver process inclusive of all available
services, the purpose behind the Person Centered Planning/Individual
Support Plan process, specifically identifies individual
skills/interests based on personal choice and the transitioning
of consumers into a semi-independent living program. TradeWinds
also experiences a positive relationship with all waiver
providers in the immediate area, helping to meet the purpose
of striving to meet the wants and needs of each individual receiving
waiver services.
